The immediate priorities when a Patient Safety Incident is identified are:

• stabilize the clinical condition of affected patients
• act to prevent further harm to affected or other patients
• act to prevent the incident happening again

An initial report should be submitted as soon as these steps are complete. Do not wait for an investigation of the incident to be completed, as support for this may be required from Coordination or Headquarters.
